Please submit changes of your personal data, such as address and name, either by post, stating your insurance number and enclosing a copy of your certificate of change of address, your identity card, your marriage certificate or a certificate of change of name. It is also possible to send the data by e-mail, attaching the aforementioned documents. However, we would like to point out that this is not a secure transmission method and that this data could be accessed by unauthorized persons.
