Change personal data
The protection of your personal data is very important to us, therefore we cannot accept address or name changes without clear identification from you via this contact form or by telephone. Please submit changes of your personal data, such as address and name, either by post, stating your insurance number and enclosing a copy of your certificate of change of address, your identity card, your marriage certificate or a certificate of change of name. It is also possible to send the data by e-mail, attaching the aforementioned documents. However, we would like to point out that this is not a secure transmission method and that this data could be accessed by unauthorized persons.
